Continuous Home Care provides short-term, around-the-clock nursing support during a period of acute symptom crisis, such as uncontrolled pain, severe breathing difficulty, or agitation. This higher level of care is designed to stabilize symptoms at home and help avoid an unnecessary hospital admission.

A Lakewood Hospice nurse or aide remains at the bedside for a majority of a 24-hour period for as long as the crisis continues, working closely with your physician to adjust the care plan. Once symptoms are under control, we transition back to Routine Home Care so your loved one can continue healing in familiar surroundings.
