In‑Home Hospice Care

About Hospice Care

At Lakewood Hospice, we bring care, comfort and dignity to you or your loved one’s own home. Our comprehensive service is designed to support both the individual and their family through this important time, delivering skilled medical oversight, personal assistance, and emotional and spiritual support—all in the place where you feel safest.

What We Offer

  • Symptom & Comfort Management — Our skilled nursing team works with you and your physician to monitor and manage pain, breathing difficulty, nausea, fatigue, and other symptoms. The aim is to ease discomfort and help maintain quality of life.

  • Daily Living Support — From bathing, dressing, and meal preparation to light housekeeping and medication reminders, our aides help with everyday tasks so that the person we care for can remain at home with dignity.

  • Emotional, Social & Spiritual Care — We believe care goes beyond the physical. Our team includes social workers, trained volunteers and chaplains who are here to provide companionship, adjust to changing needs, and support you and your family emotionally and spiritually.

  • Family & Caregiver Support — You’re not alone. We coach and partner with caregivers, offering education, resources and short‑term respite so you can take care of yourself too.

  • Flexible Care in Your Home — Whether you’re choosing routine home care or require more frequent support as needs change, we tailor our plan to you. Our goal is to help you stay in the familiar surroundings of home for as long as possible.

Why Choose Home-Based Hospice

Receiving hospice care at home means staying surrounded by what matters: your memories, your comforts, and your family. Studies show that receiving care in a familiar, peaceful environment improves emotional well‑being and decreases unnecessary hospital visits.

Our in‑home approach puts you at the center. We come to you. We listen, adjust, respond, and walk with you through each step.

Frequently Asked Questions

What services does Lakewood Hospice provide at home?

We offer full in-home hospice care, including skilled nursing, pain and symptom management, emotional and spiritual support, daily living assistance, and family education. All care is personalized to meet your loved one’s needs in the comfort of home.

How quickly can care begin after we contact you?

In many cases, we can start care within 24–48 hours. Our team will guide you through the next steps, complete any needed paperwork, and coordinate with your doctor to begin care as soon as possible.

Will hospice care cover medical equipment and medications?

Yes. Most hospice care includes coverage for medications, supplies, and equipment related to the diagnosis—such as hospital beds, oxygen, walkers, and more. We’ll handle the coordination and delivery to your home.

How often will nurses or caregivers visit our home?

Visit frequency depends on your needs and can be adjusted over time. Nurses typically visit several times per week, while home health aides may come more often to assist with daily care. Emergency support is always available by phone.
